Survey Executive Job Overall Purpose of the role:

• The Incumbent will visit various existing and Prospective SLA Partners to assess and gradehealth care providers for approval of outpatient benefits package standards.

Minimum Requirements

• KSCE Certificate
• Computer Literate
• Ensure that all service providers fill the entire form for each of the facilities to be considered for the provision of outpatient services to the Members and dependents of the client.
• The Survey Executives will be assigned a specific Region / area and would be given a list of Prospective Service Providers, major roles will include;
• Visits intended SLA partners to get addendum or new contracts
• Tally the services in SLA checklist and document the same in a checklist Form
• Meet daily targets and submit completed SLA’s forms from a given County
• Daily reporting to assigned team leaders.
